Each company may submit a maximum of three nominations, but will need to send through separate nomination forms for each entry.

Nominations can also be submitted as hard copy or on CD, by mail or courier to the addresses below:

Please note four copies of each different nomination is required for entries submitted as hard copy or on CD. This is for the benefit of our judges.

Checklist

Have you:

• Completed all company details on page one of the nomination form?
• Completed all questions on the nomination form required for the award(s) for which you are nominating?
• Included up to six (6) photographs, diagrams, maps etc of the project, product or employee?
• For hard copy or nominations on CD, included 4 copies of each nomination submitted? (If you have entered three categories you need a total of 12 entries).
